Has the U.S. Department of the Treasury, Bureau of the Fiscal Service, contacted you about a federal student loan you owe? Below are various forms to assist you.

Administrative Wage Garnishment (AWG) Request for Hearing Form – You should use this form when you have been notified that administrative wage garnishment will be started for your account or when your wages have been garnished and you wish to have a hearing regarding this action.

Financial Disclosure Statement for Wage Garnishment Hearings Form – You should use this form to provide us with your income and expense information when you have requested an administrative wage garnishment hearing for the reason that a garnishment amount of 15 percent of your disposable pay would cause you and your dependents a financial hardship.

Financial Disclosure for Reasonable and Affordable Rehabilitation Payments Form – You should use this form to provide us with you your income and expense information in order to support the establishment of a rehabilitation agreement.

Preauthorized Debit Account Application Form – You should use this form to submit account information so that you can establish automatic withdrawals (debits) from your account with the U.S. Department of Education on a regular basis.

Rehabilitation Checklist – Fiscal Service will mail this form to a borrower that enters into a rehabilitation agreement. This checklist assists the borrower in keeping track of each of the steps required to complete the rehabilitation process.

There are two versions of this checklist and Fiscal Service will send to the borrower the appropriate checklist based upon the type of rehabilitation agreement setup.

Fiscal Service will send the borrower the "15% Rehabilitation Checklist- Version 1" if the rehabilitation agreement payment is calculated using a 15% formula as described by the Customer Service Representative.

Fiscal Service will send the borrower the "Financial Information Statement (FIS) Rehabilitation Checklist- Version 2" if he/she is not able to afford the monthly amount calculated using the 15% formula. If you are documenting your expenses to reduce your monthly payment, please use this form.
